HOSPITAL REGULATIONS OF THE VETERINARY CLINIC Zviropolis Centrum Weterynaryjne

1. Admission to the hospital run by the Veterinary Clinic “Zviropolis Centrum Weterynaryjne” (the Hospital) is conditional upon the Animal Keeper signing the consent to inpatient treatment at the Hospital, the form of which is annexed to these Regulations, accepting the estimated cost of treatment, paying the fee for the animal’s first day of stay at the Hospital and accepting these Regulations.
2. The Animal Keeper of a hospitalised animal shall be deemed to be the person in whose name that animal is registered in the electronic system of the Veterinary Clinic “Zviropolis Centrum Weterynaryjne” (the Clinic).
3. In order to pass on information about the hospitalised animal, a veterinary surgeon or a member of the Clinic’s staff will telephone the Animal Keeper at least once a day. Contact with the Animal Keeper may be attempted within the following hours: 10.00-20.00.
4. Observing the contact rules set out above will allow the Hospital to give the Animal Keeper enough time during the call and to pass on all the necessary information.
5. Information about the health of a hospitalised animal will be given to the Animal Keeper or to a person authorised in writing to take decisions on their behalf.
6. All information about the cost of the animal’s inpatient treatment at the Hospital is available at the Clinic’s reception (in person or by telephone) or from the veterinary surgeon in charge of the treatment. As part of the contact referred to in point 3 above, the Hospital will, at the Animal Keeper’s request, inform them of the current balance of their account.
7. Should the animal’s health deteriorate suddenly, or should circumstances arise that call for diagnostic and clinical procedures not foreseen earlier and that will generate costs exceeding the amounts planned previously, the Clinic’s staff will contact the Animal Keeper as soon as possible in order to report the situation and obtain consent to carry those procedures out. Where contact with the Animal Keeper is not possible, the decision is taken by the veterinary surgeon, guided by the animal’s welfare.
8. The treatment and feeding plan is set by the veterinary surgeon in charge. A hospitalised animal is under the round-the-clock care of the duty veterinary surgeon and the Clinic’s support staff.
9. The Clinic accepts no liability for items left at the hospital together with the animal (leads, blankets, toys, etc.).
10. Dogs staying at the hospital are taken out for walks at least 2 times a day, unless the veterinary surgeon in charge or the duty veterinary surgeon instructs otherwise.
11. Hospitalised cats have access to individual litter trays.
12. Animals that are particularly aggressive, uncooperative or carrying infectious diseases are not admitted to the Hospital. In addition, in the case of dogs, dogs without a valid rabies vaccination are not admitted.
13. Hospitalisation costs must be settled on an ongoing basis, no less frequently than every 2 days of the animal’s stay at the Hospital.

15. If the costs of the animal’s stay at the Hospital are not settled on an ongoing basis in accordance with the Regulations, the animal will be discharged from the Hospital.
16. If the animal is abandoned, the Animal Keeper will be charged the full cost of the animal’s stay and treatment at the Hospital.
17. In the event of the animal’s death at the Hospital, the Animal Keeper is obliged to decide what is to be done with the body (collection, communal cremation, individual cremation) within 24 hours. After 24 hours the body is sent for communal cremation at the owner’s expense. In the event of the animal’s death, any items left behind may be collected within 24 hours, after which they are disposed of.
18. Hospitalisation costs do not cover medicines and materials issued to the Client to take home.
19. The veterinary surgeon in charge decides when hospitalisation ends.
20. Animals staying at the hospital may be discharged by the veterinary surgeon in charge or the duty veterinary surgeon before the agreed date only after all recommendations have been discussed in detail beforehand and the veterinary surgeon has set a date for a follow-up visit.
21. Suggestions and comments on the quality or the manner of provision of the veterinary services delivered by the Clinic’s veterinary surgeons and the support staff reporting to them should be sent to info@zviropolis.pl.
22. Disputes will be resolved on the basis of the Regulations and the applicable law.
23. The Clinic reserves the right to amend the Regulations for important reasons, namely:
a) a change in the law or the issue of final and binding administrative decisions or judgments of the ordinary courts addressed to the Clinic and requiring changes to be made, or a change in the interpretation of such legal provisions as a result of court judgments, decisions, recommendations or guidance issued by the offices or authorities competent in the given field,
b) technical or organisational changes to the services provided by the Hospital and to their scope,
c) the analysis of complaints concerning the content of the Regulations,
d) organisational and administrative changes in the running of the Clinic or the Hospital,
e) changes to the price list of the services and products offered as part of the Hospital’s activities;
f) changes in the clinical, medical and therapeutic approach resulting from the findings of clinical studies and the latest scientific research.
24. All matters not governed by these Regulations are governed by the Clinic’s regulations, made available to the Animal Keeper to read and accept. Those regulations set out, in particular, all information on the processing of Animal Keepers’ personal data in accordance with Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, GDPR) (OJ EU L of 2016, No. 119, p. 1, as amended).
