Feline panleukopenia (feline distemper): recognising, treating and preventing this deadly parvovirus

Panleukopenia — what is feline distemper?

“Feline distemper” — the name rightly worries cat owners. Panleukopenia, often known informally as feline parvovirus, is a highly contagious and potentially fatal viral disease. It is one of the most dangerous viruses your pet is ever likely to meet.

It is caused by feline panleukopenia virus (FPV — Feline Parvovirus), a close relative of canine parvovirus (CPV-2). The name of the disease — panleukopenia — comes from the virus’s main effect: a sharp drop in the number of white blood cells (leukocytes). That leaves the animal’s immune system drastically weakened. Remember: the disease moves very fast and calls for immediate veterinary treatment.

How can a cat catch feline distemper? Causes and risk groups

FPV is remarkably resistant to environmental conditions and can survive in its surroundings — on floors, bowls, hands or clothing — for many months, even years. That durability is what makes it so contagious.

Routes of infection

Infection usually happens in one of two ways:

  • Directly (faecal-oral): through contact between your cat and the faeces, vomit or secretions of an infected animal.
  • Indirectly: because the virus is so hardy, it can be carried in on your clothes, your shoes or grooming equipment, with no direct contact with a sick cat at all.

It is also worth knowing that the virus spreads through faeces, saliva and all body fluids, and even via the fleas of infected cats. It travels through the air and settles on everything in the home, which means that if several kittens live together, the disease will almost certainly reach the whole group. This is why isolating a sick cat is so important.

Who is most at risk?

The disease can strike a cat of any age, but the most vulnerable are:

  • Young kittens (under 1 year old).
  • Unvaccinated cats and those with weakened immunity (strays, for example).
  • Important: kittens infected in the womb or very early in life may suffer permanent damage to the cerebellum, leading to ataxia (an unsteady gait) and problems with coordination.

The characteristic clinical signs of panleukopenia

Signs of panleukopenia usually appear after an incubation period of 5–9 days. The disease may take a peracute course (often fatal before any signs show) or an acute one.

Panleukopenia comes in several forms, which differ in prognosis and in how severe the signs are:

  • Peracute form: the disease strikes suddenly, and kittens often die within a matter of hours, with virtually no chance of survival.
  • Acute form: the most common presentation, and one that requires immediate veterinary treatment.
  • Mild form: mainly affects older cats that have already built up partial immunity. The signs are milder (apathy, loss of appetite) and this form is easier to treat.
  • Atypical form: the signs are vague, or there is only one of them, which makes the picture hard to read.

If you see any of the following in your pet, act at once — the disease progresses at lightning speed:

  • Sudden apathy and weakness: your cat becomes extremely listless and will not move or play.
  • High fever: body temperature rises and may then fall sharply.
  • Complete loss of appetite: total refusal of food and water.
  • Severe vomiting: persistent vomiting, often frothy and yellow.
  • Acute diarrhoea: severe, bloody diarrhoea (though it is not always present).
  • Rapid dehydration: sets in very quickly because the cat is not drinking and the gut is severely affected.
  • Abdominal pain: watch for your cat adopting an unnatural, hunched posture — a sign of severe pain.

Remember: vomiting, diarrhoea and a complete loss of appetite are warning signs. Dehydration combined with panleukopenia (the absence of white blood cells) puts your cat’s life at risk.

Diagnosis and treatment: time is everything

An early diagnosis of panleukopenia is critical to survival. Working from the history (vaccination status, contact with other cats) and a clinical examination, the veterinarian moves straight on to urgent further testing.

Diagnosis

  1. Full blood count: the key finding is a dramatic fall in white blood cells (panleukopenia), often together with a fall in platelets.
  2. Rapid ELISA test: in-house rapid tests are available that detect FPV in a faecal sample. Their result still has to be interpreted alongside the clinical signs and laboratory findings.
  3. Biochemistry: to assess the damage done to internal organs by dehydration and infection.

Treatment

Because panleukopenia is a viral disease, there is no drug that attacks FPV directly. Treatment is therefore purely symptomatic and supportive, and it demands intensive in-patient care.

  1. Aggressive fluid therapy: intravenous fluids to counter severe dehydration and electrolyte imbalances.
  2. Antibiotics: broad-spectrum cover is essential to prevent the secondary — and often fatal — bacterial infections that threaten an animal with a compromised immune system.
  3. Nutritional support and symptom control: this means controlling the vomiting and diarrhoea and providing nutritional support (often by feeding tube or parenterally) when the cat goes without food for a prolonged period.

Prognosis

The prognosis ranges from guarded to poor, particularly in kittens. Mortality in untreated kittens can reach as high as 90%. Remember, though, that intensive veterinary care can greatly improve your cat’s chances of pulling through.

Prevention is everything — vaccination and disinfection

Prevention is the single most effective defence against panleukopenia. Given the high mortality and the extraordinary contagiousness of FPV, preventive measures are absolutely essential.

Vaccination

Prevention comes down to vaccinating regularly against panleukopenia. The vaccine is a core part of every cat’s schedule from 8–9 weeks of age. It is the best thing you can do for your pet’s health.

Isolation and disinfection

  • Isolation: if your cat is ill, it must be kept away from other animals.
  • Disinfection: the panleukopenia virus is extremely resistant to most ordinary disinfectants. It can persist in a contaminated area for up to a year, so you need products proven effective against parvovirus.

Is it a risk to people?

People are not susceptible to FPV. You cannot catch feline distemper. It is worth remembering, however, that we can carry the virus on our clothes, shoes and hands, acting as a vector of infection for other, healthy cats.

Feline panleukopenia is a genuine race against time. Watching your pet closely and reacting quickly to worrying signs — above all sudden apathy, loss of appetite and vomiting — can save its life.
